Liquid diets: clear liquid and full liquid
A clear liquid diet gives fluid and a little energy for a day or two; a full liquid diet adds milk, egg and strained cereals and can meet needs.
Aim and principles of the diet
Aim: To plan, prepare and calculate a clear liquid and a full liquid diet for a patient after abdominal surgery. Clear liquid diet: It gives fluid and electrolytes with some energy from sugar, leaves almost no residue in the gut and needs very little digestion. It is used as the first feed after surgery, before bowel tests such as colonoscopy, and in acute vomiting or diarrhoea. It is inadequate in energy, protein and most nutrients, so it is used for only 1-3 days. Full liquid diet: It includes all clear liquids plus milk and foods that are liquid at body temperature: custard, strained soups, thin cereal gruels (kanji), ragi malt, lassi and ice cream. Planned with milk, skimmed milk powder, egg, cereal flours, sugar and oil, it can give about 1500 kcal and 50-60 g protein. It is the step between clear liquids and a soft diet, and it suits patients who cannot chew (after jaw or mouth surgery). Both diets are given as small feeds every 2 hours so the stomach is not overloaded. Thin liquids are easily aspirated by patients with swallowing problems; they need thickened feeds planned with a speech-language therapist.

Worked calculation: Mrs. S's full liquid diet
Mrs. S, 30 years, 160 cm, 56 kg, is on the third day after an appendix operation and has moved from clear liquids to full liquids. Find her IBW, energy and protein needs and check the 9-feed menu.
- IBW = (160 − 100) × 0.9 = 60 × 0.9 = 54 kg.
- Energy = 54 kg × 28 kcal/kg = 1512, about 1500 kcal for the first days (raise towards 30-35 kcal/kg as she eats more).
- Protein = 54 kg × 1.0 g/kg = 54 g, about 55 g, for wound healing.
- Fluid = about 35 mL/kg × 54 = 1890 mL, about 2 L (nine feeds of about 200 mL plus water).
- Approximate values used (kcal and g protein per 100 g raw unless stated; rounded from Indian food composition tables, mainly IFCT 2017 and the older Nutritive Value of Indian Foods; a few, such as toned milk, are estimates): toned milk 60 and 3 per 100 mL; sugar 400; ragi 320 and 7; skimmed milk powder 357 and 38; fruit juice 45 and 0.5 per 100 mL; moong dal 325 and 24; oil/ghee 900; rice 355 and 8; buttermilk 15 and 0.8 per 100 mL; egg 75 kcal and 6.5 g per piece; cornflour/custard powder 350 and 0.3; mixed vegetables 30 and 1.5; rava 345 and 10.5.
- Cereals (ragi 20 g, rice 30 g, cornflour/custard powder 20 g, rava 25 g): 326.8 kcal, 6.5 g protein.
- Pulses (moong dal 30 g): 97.5 kcal, 7.2 g protein.
- Milk and milk products (toned milk 750 mL, skimmed milk powder 25 g, buttermilk 200 mL): 569.3 kcal, 33.6 g protein.
- Egg (1 egg): 75.0 kcal, 6.5 g protein.
- Vegetables (mixed vegetables 50 g): 15.0 kcal, 0.8 g protein.
- Fruits (fruit juice 200 mL): 90.0 kcal, 1.0 g protein.
- Oil and ghee (oil/ghee 15 g): 135.0 kcal.
- Sugar (sugar 45 g): 180.0 kcal.
- Energy = 326.8 + 97.5 + 569.3 + 75.0 + 15.0 + 90.0 + 135.0 + 180.0 = 1488.6 kcal.
- Protein = 6.5 + 7.2 + 33.6 + 6.5 + 0.8 + 1.0 = 55.6 g.
- Compare with the prescription: energy 1488.6 ÷ 1500 × 100 = 99%; protein 55.6 ÷ 55 × 100 = 101%.
- Each group is rounded to one decimal and the totals are the sums of those rounded figures; all values are approximate.
IBW 54 kg; prescription about 1500 kcal and 55 g protein. The full liquid menu gives about 1489 kcal (99%) and 56 g protein (101%), so it is adequate.
Preparation and serving tips, and result
- Strain every feed through a fine sieve; a full liquid feed should pour easily from a cup.
- Cook the egg into the custard; never give raw-egg drinks to patients.
- Stir skimmed milk powder into milk and malt to raise protein without raising the volume.
- Serve warm (not hot) in small cups every 2 hours, and note the amount taken on the intake chart.
- Clear liquid day: 150-200 mL feeds of strained juice, coconut water, dal water, clear soup, barley water and glucose water; move on within 1-3 days.
- Result: A full liquid diet giving about 1489 kcal and 56 g protein was planned; it meets Mrs. S's prescription of 1500 kcal and 55 g protein.
In the diet kitchen, liquid feeds go out on a 2-hourly round in labelled, covered flasks or jugs. A clear liquid order often comes right after surgery; the kitchen should be ready to move the patient to full liquids and then a soft diet quickly, because days on clear liquids alone cause weight and muscle loss.
Short answer: “Differentiate a clear liquid diet from a full liquid diet.” Clear: transparent fluids with no milk or pulp, low residue, inadequate, 1-3 days. Full: adds milk and foods liquid at body temperature, and can be planned to meet needs.
Short answer: “Why are raw eggs not used in liquid feeds?” Raw eggs may carry Salmonella and patients are vulnerable, so eggs are cooked into custard before blending.
Structured question: “Plan a full liquid diet for a post-operative patient and state its principles.” Principles: liquids and foods liquid at body temperature; small 2-hourly feeds of about 200 mL; milk, skimmed milk powder, cooked egg and cereal flours for energy and protein; strained and smooth. Prescription for 54 kg IBW: about 1500 kcal, 55 g protein, 2 L fluid. Menu: tea, ragi malt, strained juice, dal soup, kanji with buttermilk, egg custard, cream soup, rava kanji with milk, milk.
Clear = you can see through it (water, juice without pulp, coconut water, dal water). Full = it fills you (adds milk, custard, kanji, malt).
